Direct Support Professional – Community Habilitation Specialist

The Community Habilitation Direct Support Professional (DSP) will provide training and support in Activities of Daily Living to individuals residing in their own homes.  The DSP provides implementation and training, in the home and in the community, according to consumer needs, wants and desires as outlined in the Community Habilitation Plan

Responsibilities include but not limited to health skills and monitoring of medication and medical issues, personal hygiene/personal care needs, domestic living skills, money management, menu planning, food procurement and meal preparation, socialization and community integration, recreation and leisure time activities, safety education and training.

The employee maintains appropriate documentation according to agency policies, completing daily notes and reports in a timely fashion. The employee demonstrates the ability to communicate effectively with consumers, families and co-workers.

 

Minimal requirements: High School Diploma or equivalent.  Valid New York State driver’s license with a clean driving record.  Must have own reliable vehicle to use during working hours. Clearance from the NYS Child Abuse Registry, Medicaid Exclusions List and OPWDD Fingerprinting.  Experience working with individuals with disabilities preferred.
