We provide two consumer satisfaction surveys online: an agency-wide general survey and a survey specific to Service Coordination consumers. Agency-Wide General Survey - All consumers of therapies and sessions at the E. J. Gavras Center may take this online consumer satisfaction survey. Service Coordination - This consumer satisfaction survey is specific to Service Coordination consumers.
